Position: Clinical Mobilization Lead - Complex Care


Type: Permanent


Location: South Midlands (Coventry, Oxford, Northamptonshire)


Salary: £50 000 per annum



The Complex Care Mobilization Lead is responsible for the planning, implementation, and development of the mobilization team in collaboration with the Head of Clinical Development and Operations Director.

This role ensures the rapid mobilization of services in alignment with mobilization plans, delivering safe and effective implementation in coordination with the Senior Operations Manager and other relevant teams.



Key responsibilities include managing a small complex care case load, providing temporary clinical oversight during clinical lead vacancies or absences, and supporting contingency planning.

Additionally, the Clinical Mobilization Lead may be involved in the recruitment, on-boarding, and development of the rapid mobilization team.



Education (Essential)



  • Active NMC pin (Adult)

  • Graduate of bachelor's in nursing, DHE or equivalent



Work Experience Requirements (Essential)



  • Previous experience in community based complex care, particularly mobilization

  • Experience working with the multidisciplinary healthcare teams

  • Delivery of Clinical training including competency sign-off



Skills



  • Ability to manage own workload and work without supervision

  • Excellent verbal and written communication skills

  • Excellent record keeping and documentation management skills

  • IT skills and experience of using Microsoft Teams



Key Responsibilities




  • Mobilization Team Management:


Responsible for the strategic planning, implementation, and development of the mobilization team, collaborating closely with the Head of Clinical Development (HOCD) and Operations Director.

This role ensures effective team formation and operational readiness, aligning mobilization efforts with organizational goals and ensuring seamless integration and performance.



  • Rapid Mobilization Implementation:


Ensure the rapid mobilization of services in alignment with mobilization plans, coordinating closely with the Senior Operations Manager (SOM) and wider teams.

This role focuses on executing deployment strategies to achieve safe and effective implementation, facilitating seamless integration and operational readiness.



  • Management of Complex Care Case Load:


This involves overseeing a limited number of complex care clients with high acuity health needs, requiring specialized attention and expertise.

The role includes developing and implementing tailored care plans to address each clients unique conditions.



  • Temporary Clinical Oversight & Contingency Support:


Offer temporary clinical oversight during clinical lead vacancies and absences, both planned and unplanned, to ensure continuous care and maintain operational continuity.

This role involves stepping in to manage clinical responsibilities and support contingency measures effectively.



  • Recruitment & Development of Mobilization Team:


Provide support for the recruitment, on-boarding, and development processes of the rapid mobilization team, ensuring effective team formation and integration to meet operational needs.



  • New Client Assessment:


Conduct comprehensive assessments of clients with complex medical needs to identify their healthcare requirements, including physical, psychological, and social factors.



  • Care Plan Development:


Collaborate with physicians, nurses, social workers, and other healthcare providers to develop individualized care plans for clients, with a focus on addressing their unique needs and goals.



  • Coordination of Care:


Serve as the central point of contact for clients and their families, coordinating all aspects of care, including medical appointments, treatments, medication management, and follow-up care.



  • Monitoring and Evaluation:


Continuously monitor client progress throughout the mobilization process, track key health indicators, and evaluate the effectiveness of care plans.

Adjust care plans as needed to optimize outcomes.



  • Healthcare Navigation:


Assist clients in navigating the healthcare system, providing guidance on available resources, insurance coverage, and community support services.



  • Education and Support:


Educate clients and their families on their medical conditions, treatment options, and self-management strategies.

Offer emotional support and counselling when necessary.



  • Data Management:


Maintain accurate and up-to-date client records, documenting all care-related activities and ensuring compliance with privacy regulations.



  • Quality Improvement:


Participate in quality improvement initiatives to enhance the delivery of care to complex clients.

Identify areas for improvement and implement evidence-based practices.



  • Interdisciplinary Collaboration:


Foster collaboration among healthcare team members to ensure seamless communication and care coordination.



  • Compliance:


Ensure compliance with all relevant healthcare regulations, policies, and procedures.
