We are Looking for a Social Worker to Join a Hospital Discharge Team

This Role Requires a Social Work Qualification with a minimum of 2 Years post qualified experience

About The Role

This hospital team assess each individual case to create and implement effective care plans in order to discharge them.

By doing this they are helping people in crisis regain control of their lives.

They use the three conversational approach and model to ensure focus on people and the networks they have around them.

Some of the duties also include managing safeguarding as well as covering pathway to beds plans.

About You

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 2 years' experience is essential to be considered for the role.

Experience working with adults in a hospital setting is necessary for the role.

A valid UK drivers' licence and vehicle is essential to the success of the role.
