Social Care Worker - Make a Difference in Adult Lives

Are you passionate about improving lives? Do you want a career where every conversation could change someone's future? Worcestershire County Council is looking for dedicated and compassionate Social Care Workers to join our Adult Social Care team.



What You'll Be Doing:

You'll be on the frontline of adult social care, supporting individuals and their carers to live independently, safely, and with dignity.

Using a person-centred, strengths-based approach, you'll:




  • Carry out and review Needs Assessments and Care & Support Plans




  • Promote wellbeing, re-ablement, and independence




  • Liaise with NHS teams, voluntary services, and community partners




  • Apply key legislation including the Care Act 2014 and Mental Capacity Act 2005




  • Identify eligibility for NHS Continuing Healthcare (CHC) and advocate for service users




  • Support carers and promote community-based, cost-effective care solutions

Qualifications & Experience:

  • Experience in adult social care or community-based support

  • Knowledge of relevant legislation (Care Act, Mental Capacity Act, CHC process)

  • Strong assessment and planning skills

  • Social Work qualification not required - this role is open to non-qualified practitioners too
