Responsibilities
Provide guidance and practical support to care leavers, assisting them in becoming self-sufficient in line with the Children and Leaving Care Act, Volume 3 (1989).
Conduct thorough assessments and develop pathway plans that reflect the unique needs and goals of each young person.
Act as a trusted advisor and mentor, fostering positive relationships with care leavers, offering support in various areas of their lives, and helping them build skills for a successful transition to independence.
Advocate for the young people you work with, upholding corporate parenting responsibilities and maintaining high aspirations for their achievements.
Requirements
Demonstrable experience working with care-experienced young people, particularly in assisting their transition from being cared for to independence.
Strong assessment and planning skills aligned with pathway planning requirements.
Excellent communication and interpersonal skills, with the ability to build trust and provide empathetic, solution-focused support.
Knowledge of corporate parenting responsibilities and a commitment to achieving the best possible outcomes for young people leaving care.

This role is within the Referral and Assessment Service.
The Referral and Assessment Teams receive all new referrals from the Bromley MASH and undertake Child and Family assessments to determine how best children's needs can be met.
