Complex Needs Move On Worker

Corus Consultancy are currently looking for experienced Complex Needs Workers to work in the Lambeth area on behalf of a well established client.

KEY RESPONSIBILITIES

CLIENT SUPPORT

• To carry a caseload of eight clients mostly with complex needs including substance use problems or a dual diagnosis.
  
  To lead on assessment and development of safety and well being plans for those on the caseload and independent living action plans


• To advise residents on issues relating to good health practice, mental health and safer drug and alcohol use, the reduction of drug and alcohol related risk and harm and recovery, in 1:1 and group sessions


• To facilitate the participation of the clients in Employment and Training Schemes, the resettlement process, day-care and other programmes, and to assist in relevant activities aimed at improving life and social skills for complex needs clients.

TEAM SUPPORT

• Identify the need for targeted interventions and workshops and lead on its delivery


• Utilise knowledge skills and experience to recognise difficult or challenging client situations and use appropriate communication skills to influence and bring about positive change to the client's behaviour.

RELATIONSHIP MANAGEMENT

• Identify, develop and maintain external relationships with statutory and non-statutory service providers to increase accessibility to support for residents with complex needs, including in-reach satellite support and group work


• Arrange and convene multi-agency case conferences as appropriate; supporting colleagues to develop effective strategies to ensure clients are linked into appropriate support, attend and represent the service at different forums including the local Clinical Practitioners' group

Experience

• Experience of working with people with complex needs such as substance misuse/alcohol misuse problems and/or mental health.


• Experience and ability to build a rapport and develop a good working relationship with clients who are using substances, some of whom may display challenging behaviour.


• Experience in delivering group work and training and / or ability to learn group work and training facilitation skills.


• Must have an Updated Enhanced DBS
