Support Worker needed with Cornwall Council

Start Date- ASAP

Location - Truro

Duration - Initially 6 months

Hours per week - 37 hours per week

Salary - £13.06 ltd an hour

Description:

• To work within the Mental Health Review Team within Adult Social Care.


• To review and/or reassess the social care needs of a number of residents in receipt of packages of care being transferred from the Mental Health Trust to Adult Social Care following the ending of the S.75 arrangements


• To work within the needs assessment, and contribute to the care plan and person centred plan to enable and empower individuals to make informed choices and achieve personal goals

What is required?

• Level 2 or above e.g.
  
  NVQ or Diploma in Health and Social Care or Promoting Independence


• Experience within a similar role
