Care Coordinator

Location: Leeds, West Yorkshire

Salary: £22k - £24k pro rata + Excellent Benefits

Job Type: Part-Time, Permanent

The Client:

Our client, a reputable national franchise in the care sector, assists adults needing support to maintain independence in their private residences.

The Role:

As a Care Coordinator, you will work closely with clients, families, and carers deliver care and supervise care assistants for effective team performance.

Responsibilities:



* Update and revise clients care plans and risk assessments.


* Monthly documentation and analysis of care outcomes.


* Consult with clients for care plan recommendations.


* Perform monthly checks on care equipment as outlined in care plans.


* Maintain precise records and reports.


* Fulfil on-call duties as per the assigned roster.


* Hold monthly meetings with the care manager.


* Adhere to established policies, procedures, and practices.


Requirements:



* Previously worked as a Care Coordinator or in a similar role.


* At least 1 year of caring experience.


* NVQ Level 3 in health and social care or working towards.


* Strong computer and administrative skills.


* Valid driving license and own vehicle.


* Right to work in the UK.
