About the team
The team deals with vulnerable adults in the community to review existing care plans and packages based off CHC checklists, DST assessments and MCAs to ensure the right level of support and funding is given.
Responsibilities include assessing service users under the Care Act 2014 as well as bringing cases to commissioning panel when necessary.

About you
