I am recruiting for outreach Support Workers who have experience in supporting adults with Learning disabilties in the Norhwich and Winsford area.
You will be providing support to service users with learning disabililtes out in the community travelling around Northwich and Winsford.
A successful candidate will have experience working with adults or children and young people with emotional Behaviour Disorders, and/or disabilities and challenging behaviours.
You will have empathy, compassion, and resilience working with adults with mental health, anxiety, and complex needs.
Shift Times:
Shifts will deffer from 10:00am-22:00pm to 10:00am-18:00pm
You must be have access to your own vehicle and be able to travel around Northwich and Winsford.

Requirements:

Ability to chair meetings, review progress, and adapt plans to changing family needs.

We are looking for a Hospital Social Worker to join an experienced team of practitioners.

The fast-paced team works to independently undertake cases to facilitate safe discharges in a timely manner.
