This role is pivotal in ensuring that children and young people with SEND receive the tailored support they need to thrive and reach their full potential.
Job Purpose:
As a SEND Case Worker, you will manage a caseload of children and young people with SEND, ensuring that their educational, health, and care needs are effectively assessed, planned for, and reviewed.
You will work closely with families, schools, and other agencies to develop and implement Education, Health, and Care Plans (EHCPs) that reflect the individual needs of each child, ensuring compliance with legal requirements and best practices.
Key Responsibilities:
Case Management: Manage a caseload of children and young people with SEND, overseeing the entire process from assessment to the creation, implementation, and review of EHCPs.
Assessment and Planning: Conduct thorough assessments to identify the needs of children and young people with SEND, working with families, schools, and professionals to develop comprehensive and person-centred EHCPs.
Collaboration: Liaise with a range of stakeholders, including parents, schools, healthcare providers, and other agencies, to ensure a coordinated approach to meeting the needs of children and young people with SEND.
Compliance: Ensure that all EHCPs comply with statutory requirements and reflect the latest guidance and best practices.
Monitor the progress of plans and make necessary adjustments to meet changing needs.
Support and Advocacy: Provide support to families, ensuring they understand their rights and options within the SEND framework.
Act as an advocate for children and young people, ensuring their voices are heard in decision-making processes.
Monitoring and Review: Regularly review EHCPs to assess their effectiveness and make recommendations for changes where necessary.
Ensure that all records and documentation are accurate, up-to-date, and compliant with data protection regulations.
Training and Development: Stay informed about the latest developments in SEND legislation and best practices.
Participate in ongoing professional development to enhance your skills and knowledge.
Key Skills and Experience:
Experience: Demonstrable experience working within a SEND or related field, particularly in case management or similar roles.
Experience in a local authority or educational setting is desirable.
Knowledge: In-depth understanding of the SEND Code of Practice, the Children and Families Act 2014, and related legislation.
Familiarity with EHCPs and the statutory assessment process.
Communication: Excellent communication and interpersonal skills, with the ability to build positive relationships with children, families, schools, and professionals.
Strong report writing and record-keeping skills.
Empathy and Advocacy: A compassionate approach with a strong commitment to advocating for the rights and needs of children and young people with SEND.
