We are looking for an Adult's Social Worker to join a Hospital Discharge Team.
This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.
About you
The successful candidate will have worked in a hospital team or short term assessment setting.
A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience working in an Adult's Social Work environment is essential.
A valid UK driving licence and car is not essential but is preferred for this role.
About the team
This is a fast paced team who work to ensure patients are safely discharged from hospital and have enough support as they need.
Carrying out bedside assessments, arranging appropriate care packages for comprehensive discharges and advocating for service users and their independence is at the forefront of the team's motivation.
Being able to ensure service users have access to necessary resources and external support at the earliest stages is key to the success of this role.

We are seeking enthusiastic, experienced support workers to support our client.
Our client supports Adults across Somerset with Autism, and Learning difficulties in a supported living setting.
In Bridgwater we require support in a 5-bed and a 7-bed home, working with adults with learning difficulties and behavioural needs.
Location: Bridgewater
Shifts: 7:30-14:30 / 14:00-22:00, 8:00-14:00 / 14:00-20:00, Alternate weekends
Rate of Pay: £12.21 + holiday pay PAYE, or £16.05 Umbrella
Requirements:
Driver, essential (You will not be required to use your own vehicle)
Experience working in a supported living environment or similar
Experience with Autism and Learning Disabilities
Must be able to work full-time, including alternate weekends

Agency School Nurse (Learning Disability/Paediatric equivalent to NHS Band 5) required to work on an Agency Basis, Monday to Friday 08:30-16:30 on an ongoing basis, starting ASAP!
Our client is both a school and a registered children's home for children who have severe and complex learning difficulties and autism.
This position is an immediate start (once compliance checks are completed) You will be working with a small team of four.
On a day-to-day basis you will collectively oversee the provision of all health care needs of the children.
You will be required to liaise with other health professionals, and multidisciplinary team members within the organisation to fulfil duties, including:
Supervising visits with health professionals and assisting our children to hospital appointments and surgeries in the community
Observing, assessing, and monitoring the children's health, development, and emotional wellbeing, initiating action when necessary
Contacting parents concerning the health needs of their children
Supporting with staff training in the dispensing of medication, epilepsy, and associated risk assessments
Offering advice and treatment for minor injuries.
Ordering medication from the GP and validating through internal systems once collected from the pharmacy
Pay
PAYE Starting point £21.00 plus holiday pay
Umbrella Starting point £25 per hour
