This service aim to support patients in a range of community-based activities as part of a full treatment programme and to prepare them in moving through their discharge pathway towards a community based placement

As a Nurse your key duties include:
Deliver prescribed care to a defined group of clients/patients within an agreed framework
Plan and implement a structured approach to nursing care which takes account of patient choice and wishes
Accurately observe and engage with patients in order to assess their needs and to evaluate progress
Monitor and review Nursing Care plans
Evaluate and respond to changes in patients condition and needs
Deal proactively and effectively with suggestions and complaints from patients and their families/carers
Contribute to a team approach to patient care in conjunction with all members of the Multi-Disciplinary team
Participate in regular clinical and non-clinical meetings and attend other meetings as requested
The following skills and experience would be preferred and beneficial for the role:
Knowledge of NMC guidelines and professional practices
Ability to develop and use flexible and innovative approaches to practise
Excellent verbal and written communication skills
Willingness to actively achieve Preceptorship standards and competences
The successful Nurse will receive an excellent salary of £32,760 per annum.

As the Senior Staff Nurse your key duties include:
Ensure that all patients within the ward have their needs assessed, that programmes of care are developed to meet those needs, and are delivered in accordance with agreed policy during their span of duty
The post holder will assist the Ward Managers in the evaluation of care offered by nursing staff on nights
Ensure that staff complete the nursing records for each patient in accordance with agreed policy
Ensure that advice/support and information to patients and their relatives/friends is provided as required
Evaluates the effectiveness of care within the ward and takes appropriate action if required
To facilitate liaison with other disciplines as necessary to meet the needs of patients
Responsible for setting the standards of care, at nights, to be achieved by example and direction
The following skills and experience would be preferred and beneficial for the role:
Relevant clinical experience
Demonstrates a positive attitude and commitment to change, improvement and quality
Ability to develop and use flexible and innovative approaches to practise
Excellent verbal, interpersonal and written communication skills
The successful Senior Staff Nurse will receive an excellent annual salary of £40,104.36 - £42,704.30 per annum DOE.
This exciting position is a permanent full time role working 37.5 hours a week.

As the Ward Manager your key responsibilities include:
Primary nurse responsibility for an allocated caseload of patients
Plan and implement nursing care of individual patients paying particular attention to areas of nursing risk management
Attend clinical team meetings and CPA reviews, actively participating in Multi-Disciplinary assessment and treatment planning
Assist in the co-ordination of nursing activity over a 24-hour period to include adequate rostering, grade and skill mix provision
Attend department/ward meetings as directed.

As the Senior Occupational Therapist your key responsibilities include:
Co-ordinating and managing on a day-to-day basis the effective operation of the Occupational Therapy Service, reflective of agreed contact targets
Leading on the planning and delivery of multi-disciplinary therapeutic patient activity programmes across the hospital
Provide clinical support and professional supervision to the occupational therapy team
