About the team:
This team operates within a multidisciplinary environment to provide specialist support to adults with learning disabilities and their carers.
Main responsibilities will include holding a caseload of adults, offering support to individuals to live as independently as possible and ensure safeguarding processes are caried out in line with the Care Act 2014.
