The MDT comprises; a Diabetologists, 3 x specialist nurses and a full-time dietician and a newly created Band 4 Diabetes Technologist role.
The Service provides a specialist service across the acute hospital from the specialist 'Diabetes Hub' at the hospital, and community but there is an opportunity to either rotate or work predominantly within your preferred area.Reporting to the Lead Diabetes CNS you will:- work collaboratively with multi-professional teams in Primary Care, Secondary Care, in the community and in all care settings (including domiciliary visits) and applies research-based practice to achieve the optimum ‘best clinical practice’ in diabetes care. - act as a clinical leader for diabetes for health care workers, patients and their carers in all clinical settings.
- the role also provides clinical leadership in diabetes care by using strategies such as teaching and auditing services.
