I am looking for an Adults' Social Worker to join a Healthcare Assessments team in Manchester.
This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.
About the team
The team work in partnership with disabled adults to understand and support them in what they need.
In this role, the team use DST (Decision Tool Support) and travelling countywide.
The Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS.
About you
A degree within Social Work (Degree, DipSW, CQSW) with a minimum of 2 years' experience is required to be deemed suitable for this role.
The ability to assess the package of care for the adults is important.
