Clinical Nurse specialist / Paramedic

We are looking for an enthusiastic and dynamic Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist (CPS) to join our Outreach Team.

You will provide specialist palliative care support to patients living in the community, in their own homes, and in care homes/nursing homes.

About the Role:

Position: Band 7 Clinical Nurse or Paramedic

Specialisation: Palliative Care (Experience or Qualifications required)

Schedule: 7-day week, 8am to 6pm

Employment Type: Full-time and Part-time positions available

Location: Kent (Free on-site parking)

Start Date: As soon as available, subject to standard checks

Rates: Competitive locum rates, either PAYE or Limited company

Responsibilities Include:

- Serving as the initial point of contact for new community referrals and telephone inquiries.

- Participating in a rota of clinicians covering a 7-day service.

- Providing advice and care to patients in clinic and home settings, as well as remotely via telephone and videoconferencing.

- Focusing on individualized care, improving quality of life, and achieving patient-centered goals.

- Working alongside other community teams and specialists to optimize patient experience and enhance team knowledge and development.

To Succeed in This Role:

- Proven community or in-patient palliative care experience at Band 6 or 7, or equivalent experience in areas such as Paramedic, General Medicine, Neurology, Emergency Medicine, Frailty, Respiratory Care, Cardiology, or Oncology.

- Motivated clinician with the ability to work independently and as part of a responsive and resilient team.

- Experience in mentoring and supporting individuals and teams.

- Advanced communication skills and excellent presentation skills (both oral and written).
