About the team

The opportunity is within the Integrated Discharge Team.

The team does work across both Doncaster Royal Infirmary in the Transfer of Care Hub and across the multiple discharge pathways to determine the appropriate ongoing long-term needs.

About you

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of three years' post qualified experience is essential for this role.

You will provide a very high quality and effective service to adults and carers.

You should also be able to work autonomously, using your initiative to manage your own caseload.

Your ability to communicate, both written and verbally, needs to be effective and you will be able to use evidence base practice to take appropriate action.
