This company is seeking a skilled and experienced Community Nurse to work on a temporary agency contract within NHS Worcester.

The successful candidate will be responsible for delivering evidence-based care to patients within their home environment who have complex needs.

As a Community Nurse, you will work closely with specialist nurses, GPs, therapists and social work teams to provide collaborative support to patients and their families, and prevent avoidable hospital admissions.

• Deliver skilled and evidence-based care to patients within their home environment


• Work closely with specialist nurses, GPs, therapists and social work teams to provide collaborative support to patients and their families


• Prevent avoidable hospital admissions

• Valid NMC registration


• Previous experience as a Community Nurse


• Ability to work independently and as part of a team


• Excellent communication and interpersonal skills


• A car and driving licence is essential for this role
