We are Looking for an Advanced Practitioner to Join an Adult's Hospital Team

This Role Requires a Social Work Qualification with a minimum of 4 Years post qualified experience

About The Team

This team works in an acute hospital setting with individuals and their families to ensure that they have a safe discharge, by building a rapport with health professionals in order to maintain effective working.

They provide patients with the necessary medical referrals for recovery, coordinate home health care services, and give psychological support.

This involves dealing with people with complex care needs as well as safeguarding aspects.

This role requires supervision and guidance of Social Workers - as well as other Social Care Professionals in the team.

This is best assisted with a high level of Social Work knowledge.

About You

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 4 years' experience is essential to be considered for the role.

Experience working with adults in a hospital setting will benefit in this role as well as experience in leading and supervising social workers.

A valid UK drivers' licence and vehicle is essential to the success of the role.
