We are looking for a Social Worker for a Hospital Team

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the Team

The hospital team supports patients during their hospital stay and plans for safe discharge.

They carry out assessments of patients' care and support needs, mental capacity, safeguarding concerns, and risks, in line with relevant legislation.

The social workers helps arrange care and support at home or in the community, work closely with families and healthcare staff, and promotes independence, wellbeing, and safe transitions from hospital to the next stage of care.

About you

A degree within social work (Degree/DipSW/CQSW) with a minimum of 2 years' post qualified experience is essential in order to be considered for this role.

A valid UK driving license and vehicle is not essential to the success of this role but will help.
