This is an exciting opportunity for someone who is passionate about building strong partnerships across health and social care to ensure people can return home safely and receive the support they need.
The primary focus of this role is to work closely with Great Western Hospital (GWH) and community healthcare professionals, coordinating referrals and supporting smooth, timely transitions from hospital to home.

You will work collaboratively with a wide range of professionals including Hospital Discharge Teams, Social Workers, Occupational Therapists, GPs, District Nurses, Community Healthcare Teams, and other partner agencies to coordinate care packages, respond to referrals, and promote positive outcomes for customers living independently at home.
Alongside this, you will support the day-to-day coordination of our domiciliary care service, helping to ensure high standards of care delivery and continuity for both customers and care staff.

- Act as a key point of contact for customers, families, and care staff.
- Respond promptly to telephone calls, emails, and messages.
- Record and escalate concerns, complaints, compliments, and safeguarding issues appropriately.
- Work closely with the Registered Manager and wider care team to maintain high standards of service quality.
- Participate in the shared out-of-hours on-call rota, providing support to customers and staff while managing emergencies and operational queries.
