Adult Social Worker Hospital Discharge | West Yorkshire | £35.00 per hour| Hybrid

Charles Hunter Associates is seeking an experienced Social Worker to join a Hospital Discharge Team within a Local Authority in West Yorkshire.

This is an exciting opportunity to work in a fast-paced environment, supporting vulnerable adults to achieve safe and timely discharges from hospital back into the community.

You will work collaboratively with multidisciplinary teams and partner agencies to ensure individuals receive the appropriate care and support to meet their needs.

This is a locum opportunity; the social worker is required to have 1 year post ASYE qualified experience within a direct local authority.

Experience of carrying out Care Act 2014, Menta Capacity Act and Section 42 assessments.

Desirable experience for this post includes working within community team, hospital discharge, safeguarding hub's and/ or working with vulnerable adults that have complex care.
