We are looking for an experienced Adult's Social Worker to join the Mental Health Hospital Discharge Team in the London area

Do not apply if you do not have a Social Work qualification with a minimum of 2 year post qualified experience of working in a English local authority.

About the Team

Providing high-quality adult social care services in line with the Care Act 2014, undertaking assessments, risk assessments, and Mental Capacity Assessments.

Developing strengths-based care and support plans that promote independence, wellbeing, choice, and safeguarding.

Managing a caseload, conduct reviews, coordinating with partner agencies, and maintains accurate records to ensure statutory compliance.

Supporting individuals and carers through advice, signposting, and access to appropriate services while contributing to staff development, service improvement, and equality and diversity.

About you

An enthusiastic and committed Social Worker who can work effectively in a fast-paced environment is essential for this role and previous experience working in Mental Health Team.

Social Workers must hold a Social Work qualification (Degree/DipSW/CQSW) and have a minimum of three years' post-qualifying experience.

A valid UK driving licence and access to a vehicle are also good but not essential for the success of this role.
