About the service: Opportunities have arisen for Community Mental Health Nurses (Care Co-ordinator) within both East and West CMHTs.

The CMHT work with people aged 16 and over with severe and enduring mental health and social care needs.

The CMHTs are integrated with the local authority, providing a multidisciplinary approach to the care provided.

Care co-ordinators are responsible for providing holistic assessment, interventions, risk assessments and care plans for their caseload.



Responsibilities:

• Conducting initial and ongoing assessments of patients' mental health conditions and social circumstances.


• Crafting care plans that address medical, psychological, and social aspects of mental health.


• Monitoring the effectiveness of care plans and updating them as needed.


• Acting as a liaison between patients, healthcare providers, social workers, and other support services.


• Responding to emergencies or significant changes in a patient's mental health or living situation.

Qualifications and Requirements:

• Fully enhanced valid DBS


• Mental Health Nursing Degree required.


• Community experience is essential.

Rate Breakdown:
£27ph LTD | PAYE Inclusive: £24.29

We offer a £250 bonus for starting a contract with SCS.
