We are looking for an Adult Social Worker to join the Mental Health Discharge Team.

This role requires a social worker to have a qualification with a minimum of 2 years post qualified experience

The Role
This team works to promote a quality service to support adults of working ag who are experiencing severe mental health problems on short term interventions.

You will be responsible to undertake care act assessments of the service user and carers, prepare reports for court, formulate support plans, compete funding applications and risk assessments.

About You
A social worker must have a minimum of two years' experience and have a degree within social work (Degree/ DipSW/COSW).

The potential individual will be enthusiastic and have passion to ensure the people have the right care with the ability to work in a fast-paced environment.

A valid UK driving license is essential.
